Hybrid Home & Virtual Oncology NP - Hands-On Care

Daymark Health seeks an experienced Nurse Practitioner or Physician Assistant to join our home-based oncology care team. You will deliver hands-on clinical care in patients' homes and through virtual visits, supporting a high-performing clinical operation.

You'll work as a player/coach, triaging high-acuity patients, collaborating with oncology practices, and helping to implement our evidence-based care model in the Inland Empire region, with a focus on compassionate, coordinated care.
