divh2Licensed Practical Nurse/h2pFor over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness./ppThe Licensed Practical Nurse works as an integral member of the team providing services to senior residents who are aging in place or are clients in the Mental Health Clinics/PROS who are diagnosed with a behavioral health concern. The Licensed Practical Nurse is responsible for providing a range of wellness services to meet the behavioral health, medical, and care issues affecting those living independently within the Housing buildings and clients attending the Mental Health Clinics, Care Coordination or PROS programs./ppThe Licensed Practical Nurse will be responsible for developing and implementing practices and protocols for clients ranging the entire life span. Residents in crisis or with behavioral health needs (such as hoarding, depression, anxiety, substance use disorders) will be reached out to, engaged and treated by the Licensed Practical Nurse. Additionally, the LPN will address and assess for medical concerns presented by clients in housing and the clinics/PROS. The professional will develop an individualized plan to address needs and include family, significant others and healthcare providers whenever possible. The primary goal of this employee is to support residents to maintain permanent affordable housing or, where appropriate, aide in transitioning to more appropriate housing. Within the Mental Health Clinics/PROS and Care Coordination, the primary goal is to assess medical concerns, perform health monitoring services, and provide linkages to treatment, in addition to providing health teaching and counseling around current and future health concerns./pulliAssesses clients to establish a care plan according to their needs./liliPerform health monitoring services and assessments/liliResponsible for tracking information/ deliverables for the Cabrini grant funds./liliLead workshops for clients on issues related to health and wellness./liliEncourage clients to participate in on-going wellness and social activities./liliProvide advocacy and education to clients/liliFacilitate coordinated care for targeted critical/high need cases./liliWorks through team approach in partnership with program staff to identify and respond to clients in need./liliWorks with client and their families to implement planning./liliCompletes documentation, both paper and electronic in a timely manner./liliProvide counseling and therapy./liliProvide in-service training and be available to consult with staff./liliConduct and prepare assessments, case logs, documents case notes, care plans for clients./liliRefer and link clients to service providers in the local community./liliEstablish and maintain contacts with families and programs when clientss are in hospitals and rehab facilities, and helps formulate aftercare plans./liliResponds to emergency situations and provide crisis intervention./liliContact and follow-up with service providers put in place for clients such as Adult Protective Services, outpatient behavioral health, substance use treatment and healthcare providers./liliStrictly adheres to and enforces all the Agency policies regarding Confidentiality and Corporate Compliance regulations and maintains compliance with HIPAA regulations./liliOffer time-limited groups as appropriate on such issues as loss and depression, living with illness, substance use issues and family/relationship maintenance./li/ululliAttend staff meetings and trainings as required/liliParticipate in the development and facilitation of staff training and in-services for social service staff on relevant social service topics./li/ulpQualifications/pulliNYS Licensed Professional Nurse/liliMust have certification in behavioral health clients or 3 years of experience working with the behavioral health population./liliExcellent time management, organizational and computer skills./liliAbility to appropriately evaluate and intervene in crisis situations./liliExcellent oral and written communication skills required./liliBilingual preferred (Chinese, Korean, Russian, Spanish)/liliAbility to work flexible hours (evenings/weekends) as assigned./liliAble to travel throughout the five boroughs as needed./liliAbility to lift up to 10lbs./liliMay be required to meet with clients and / or staff throughout programs./li/ulpBenefits/pulliWe offer competitive salaries and excellent benefits, including:/liliGenerous time off (Vacation/ Personal Days/ Sick Days/ Paid Holidays annually)/liliMedical/liliDental/liliVision/liliRetirement Savings with Agency Match/liliTransit/liliFlexible Spending Account/liliLife insurance/liliPublic Loan Forgiveness Qualified Employer/liliTraining Series and other additional voluntary benefits./li/ul/div
